An Advanced Care Directive appoints a person or persons of your choice to make decisions regarding your healthcare and personal welfare if you are legally incapacitated.

Why do you need an Advanced Care Directive?

If you do not have an Advanced Care Directive, and you are legally incapacitated, your family or friends will need to apply for a court order to become your guardian. If there is a dispute between family members the Public Advocate will usually be appointed.
